Abstract

 

Objective:Because of various reasons,people’ psychological stressors of increasing,thereupon the incidence of alcohol dependence disease increases.Alcohol dependence has become an important social and medical issue in modern society.Alcohol dependent patients have negative personality characteristics and their cognition,emotion and behavior changed.Therefore,alcohol dependence treatment which only relies on psychotropic drugs cannot be effective,while it must be imposed on necessary psychological correction at the same time.At present,the Cognitive Therapy has been widely used on alcohol dependent patients at home and abroad, and the curative effect is certain.NaiKan Therapy is one of the most effective ways to treat alcohol dependence diseases.In our country,there are many scholars who applied NaiKan Therapy to alcohol dependent patients and they acquired curative effect.But most of them were individual cases and case reports,besides,psychological mechanisms related to effective treatment were not clear.This study is the first one which applies NaiKan Cognitive Therapy on larger samples of alcohol dependent patients in intervention studies,in order to assess their clinical efficacy.This is a highly operable and effective psychological intervention method which can be used in clinical treatment and especially in psychotherapy for alcohol dependent patients.Methods:The patients from March2011to March2012met the inclusion criteria and exclusion criteria,totally,64patients were selected into the study.They were voluntary to take part in this study.The informed consents were signed by all patients.All alcohol-dependent patients were randomly assigned to the study group and the control group:The study group treated with NaiKan Cognitive Therapy(NCT)for successive7days on the basis of taking original drugs;the control group only gave the original drug therapy. Obsessive Compulsive Drinking Scale(OCDS),Symptom Checklist90(SCL-90),Nurses Observation Scale for Inpatient Evaluation(NOSIE),Personal and Social Performance Scale(PSP),World Health Organization Quality of Life Brief Scale(WHOQOL-BREF)were administered to all subjects at pre-and post-treatment.The PSP was evaluated again after eight weeks. Results:64alcohol-dependent patients were recruited in the study,30subjects in study group and other30subjucts in control group were involved the result analysis,4patients draw out.Before treatment,there were no significant difference between the study group and the control group by all the assessment tools.After the treatments,the scores of OCDS in study group were lower than before,the difference was statistically significant.After the treatments,the scores of OCDS in study group were lower than that in control group,within two groups the statistical difference had the significance(P<0.01).After the treatments,the SCL-90total score and factor scores of interpersonal relationship,depression,anxiety,terror in study group were significantly lower than before,the difference was statistically significant (P<0.05).After the treatments,the SCL-90total score and the factor scores of interpersonal relationship,depression,anxiety in study group were lower than that in control group,the difference was statistically significant(P<0.05).After the treatments,the NOSIE scores of total negative factor,irritation factor,retardation factor,depression factor in study group were lower than before,the statistical difference was significant(P<0.01),the scores of total estimated factor,total positive factor,social competence factor,social interest factor,Personal cleanliness factor were higher than before,the statistical difference was significant(P<0.05).After the treatments,the NOSIE scores of total estimated factor,total positive factor,social competence factor in study group were higher than that in control group,the. statistical difference was significant(P<0.05).The scores of total negative factor,irritation factor, depression factor in study group were lower than that in control group,the statistical difference was significant (P<0.05).After the treatments and eight weeks,the scores of PSP in study group were higher than before,the difference was statistically significant (P<0.01).After the treatments and eight weeks,the scores of OCDS in study group were higher than that in control group,within two groups the statistical difference had the significance (P<0.01).After the treatments,the WHOQOL-BREF scores of quality of life,physiological domain,psychological field,social relation field,environmental field in study group were higher than before,the statistical difference was significant(P<0.01).After the treatments,the WHOQOL-BREF scores of quality of life,psychological field, social relation field,environmental field in study group were higher than that in control group,the statistical difference was significant (P<0.01).Conclusion:NaiKan Cognitive Therapy can effectively inhibit alcohol craving and reduce problems related with drinking.It can help to improve alcohol dependent patients’ psychological symptoms,especially depressed,anxious and interpersonal symptoms.Besides,it will significantly improve the level of mental health.NaiKan Cognitive Therapy can improve social function in patients with alcohol dependence, enabling them to get social support,so as to reduce the dependence on alcohol spirit.NaiKan Cognitive Therapy can improve negative cognition and emotion in patients with alcohol dependence and increase their sense of self-worth,self-confidence and self-esteem.Also,it is beneficial to improve the quality of life in patients.The influence factors of NCT on alcohol craving:the reduction of alcohol craving are closely related to the improvement of psychological symptoms,especially depressed,anxious,interpersonal symptoms and negative cognition.As a psychotherapy treatment with shorter treatment time, simplified operation and curative effect,NaiKan Cognitive Therapy has great prospects for alcohol dependent patients.
